American Family Matters, LLC (AFM) is a new agency that serves the physically and intellectually disabled of all ages who are on the I/DD waiver program in Jefferson, Berkeley and Morgan counties to achieve the highest level of independence and self-sufficiency as possible. AFM offers knowledgeable and compassionate professional staff that combined has over 25 years of experience working with the special needs population.

At AFM, each individual will be treated based on their specific needs. Services are provided in the natural setting, home setting and community setting where the individual resides, works, and shops. All individuals will have a well-qualified, trained and caring Service Coordinator who will work one on one with each individual and family to ensure needed services are being met as best as possible and provide education, linkage and referral to community resources as available. AFM also has a licensed Social Worker who serves as the Therapeutic Consultant/Behavior Support Professional who will work with each individual in development of person centered training goals and behavior plans to encourage independence with Activities of Daily Living and will provide training to the primary caregivers.
